New Patient Registration
The doctors welcome new patients who live within our N9 practice area.
Please fill in a form at the reception desk. You will be asked to complete a simple survey to help us understand your needs (patient profile).
From Monday 15/05/2017 we will be taking new registration applications on the days and times stated below
TUESDAY 11.00am - 17.00pm
WEDNESDAY 11.00pm - 17.00pm
THURSDAY 11.00am - 17.00pm
FRIDAY 11.00am - 17.00pm
Once registered, you will be given a new patient health check appointment to see a nurse. Please bring any helpful medical information with you e.g. lists of medications or your child’s Red Book.

Online 'pre-registration' with the practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.
Your registration is NOT YET COMPLETE, to be fully registered you will need to click on the Online Medical Questionnaire link below, complete all the details and press "Send" to mail your form to us.
We at Forest Road want to make it as easy as possible to register. It is always very helpful to bring Photo ID but don't worry if you don't have this you can still register. We will ask you to Sign and Date a Registration Form. Once this is complete we will offer you appointment with our Health Care assistants for a new patient Health check.
However if you wish to access your records on-line, in order to protect your confidentiality and protect your data we will require photographic proof of identity.
Please could you bring in the RED BOOK for any children you are registering.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off the online forms, fill them in and bring it in with you along with Photo id on your first visit to the practice.
